Everything you need to know before your first visit—what to bring, what to expect, and how to prepare.

Everything you need to feel prepared, comfortable, and confident for your first visit with us.
At Good Life Physical Therapy we want your first visit to feel easy and empowering. From what to wear to what to bring, we’ll guide you every step of the way so you can focus on what really matters—feeling better. Whether you’re here for a tune-up, injury recovery, or ongoing care, your comfort is our priority.
Before your appointment, we’ll ask you to fill out an intake form to better understand your concerns, history, and goals. You'll have a chance to fill out these forms before you come in-- or complete them in the office. If you have any imaging, medical records, or prescriptions related to your condition, feel free to bring them—but if you don’t, that’s okay too. Comfortable clothing that allows for movement and access to the area of concern is ideal, but there’s no need to stress—we’ll adapt to you.
Most importantly, come ready to share what you’ve been feeling and what you’re hoping to get out of therapy. We’ll take the time to listen, explain, and begin building a plan that works for your body, your life, and your vision of health. You don’t need to know anything about physical therapy—we’re here to guide you from the first step.
After we review your intake forms, you will join your therapist in one of our private treatment rooms. You will have plenty of time to discuss your medical history, what issues you are having, and what your goals are. Your physical therapist will then perform an examination which will include testing various muscles and joints as well as assessing your nervous system and functional movements. A hands on assessment of muscle tone and joint movement will also be performed. Following this complete evaluation, your therapist will discuss your diagnosis and together you will design a treatment plan that works for you. You'll have an opportunity to ask questions and will go home with a clear plan, feeling better.
